UMB Faculty Applicant Affirmative Action Information Request Form

Effective April 1, 2010, the Faculty Hiring Agent or Appointment/Promotion/Tenure (APT) contact person shall be responsible for sending each faculty applicant the link to the new UMB Affirmative Action Information Request Form located here.

The automated form replaces the (Yellow) Affirmative Action Information Request Card. The information is needed for the purpose of completing important governmental compliance reports to help evaluate the University's progress in hiring underrepresented persons in disciplines/fields or in difficult to recruit positions.

In order to complete the form, the faculty applicant should also be provided with the position number for the job in which they are applying for. The position number must be entered twice to validate they are inputting the correct number. If the faculty applicant fails to do so they will receive a message that reads -

Please make sure your position number is correctly entered. Go Back

Once the information is entered correctly, and the form is completed, the faculty applicant will click submit and the information then goes directly into our database.

Continuation of the use of the mail process of the "yellow card" is acceptable until April 15, 2010. After this date the cards should be returned to HRS immediately to be destroyed.
